Primary Home Care

The Primary Home Care (PHC) program is a nontechnical, medically related personal care service provided to adults whose health problems cause them to be functionally limited in performing activities of daily living. You will need a doctor's statement of your medical need.

These PHC agencies also provide Family Care Services. The Family Care service also is a nonskilled, nontechnical service provided to eligible clients who are functionally limited in performing activities of daily living.

Services Covered by the PHC program include:

Escort Services: You may have an attendant accompany you on trips to obtain medical diagnosis, treatment, or both. This service does not include the direct transportation for you by the attendant.

Home Management: This program provides assistance with housekeeping activities that support your health and safety, including:

  • changing bed linens
  • housekeeping
  • laundering
  • shopping
  • storing purchased items
  • washing dishes

Personal Care: Assistance with activities related to caring for your physical health, including:

  • bathing
  • dressing
  • grooming
  • routine hair and skin care
  • preparing meals
  • feeding
  • exercising
  • helping with self-administered medication
  • toileting
  • transferring/ambulating

Eligibility requirements

  • You must qualify financially as determined by the Texas Department of Aging and Disability Services.
  • You must be age 21 or older or be enrolled in the Consumer Attendant Services program.
  • You must need help with at least one personal care task.
  • You must have an unmet need for services.
  • You must have a practitioner's statement of medical need.
